Dear Admin,
Please help!  I have been trying to make a questionnaire that I can email someone to fill in.  I have been racking my brain trying to figure out how to do this.  Can you please help?  The answers to some of the questions I will be using on this questionnaire are some text and some numerical.

Hi,

Do you own a website with a mailto form setup? If so, create your email with this page (set email message to HTML) and when the email form is filled out, the website's mailto form will process it and email you back the results.

If not, let me know what email client you use (e.g. Outlook Express....)

Dear SA,
No, we do not own a website.  We do own a domain name for our email service.  We do not have an email server here in our office.  We use our ISP's server to host our email.  We do use MS Outlook 2000 for our email.

Hi,

Many ISP provide a mailto form code you can use. If your account gives you a small amount of webspace you could do what I suggested.

Is this form for internal use at work only or for your external clients?

Maybe I missed the point here! Are you simply trying to attach a Word document as a form?
